If a doctor cannot continue treatment of a ClearCorrect case for any reason, the case may be officially transferred to another ClearCorrect provider by filling out the case transfer form. The receiving doctor must be a registered ClearCorrect provider. To register, please call (888) 331-3323.
You may email the form to a support rep or you may fax it to (713) 583-9643. It is perfectly alright to use two forms if needed.
