What are the main indications for auxiliaries, what are the limitations and side effects?

Auxiliaries enhance the force applied by aligners. When a tooth is not tracking with the aligner, the auxiliaries can direct pressure in the desired direction. Limitations and side effects will vary depending on the type of auxiliary used and how it is applied.




How can I predict when I may need to use auxiliaries? I would like to be able to tell a patient that these items may be used in their treatment.

Predicting the need for auxiliaries gets easier with experience. You can tell your patient that auxiliaries may be necessary when you see difficult movements such as extreme rotations, extrusions, and inter-arch corrections.


 

How do you best use pliers in a situation where a tooth is not moving as predicted?

Pliers can be used to make "dimples" in the aligner which can help increase pressure on a tooth to help make the desired tooth movement(s) or to increase the retention of the aligner. For more information on the use of dimples, please refer to this article.




Are elastics used when the clear aligner fails to move the tooth or is it sometimes incorporated into the original treatment setup?

It's important to remember one of the basic tenets of clear aligner treatment: when a tooth is properly/fully encased in the aligner material, it cannot be moved except by the forces built into the aligner. Cutouts for elastics can be requested in "Additional instructions" when submitting a case in the Doctor Portal or in the Comments Panel of ClearPilot™. 

Elastics are effectively used in conjunction with aligners in a few scenarios:

  1. Class II elastics, Class III elastics, and vertical elastics can be used with aligners to effect dental arch (skeletal) movement. This type of elastic application should be incorporated into the initial treatment plan created by the clinician.
  2. Elastics can be used to extrude individual teeth into occlusal gaps in the aligner caused by the teeth not moving as expected. 
  3. Elastics can be used to help rotate individual teeth into the desired position if/when the rotation is not occurring with the aligners alone.

If you choose to use elastics to treat Class II or Class III bite relationships, you can request to have slits made in the aligners, cutouts in the aligners where buttons are bonded to the teeth, or a combination of both button and slit cutouts. The preference is up to the treating clinician.

How often should elastics be changed?

A general rule of thumb is to change them every time the patient removes them to eat or drink.




I have difficulty adhering buttons to crowns and to the aligner as well, any suggestions?

Due to the flexibility of the aligner material, it can be difficult to keep buttons bonded to the aligner material. However most bonding resins in the market will work well. You may want to consider using the slit cutouts option as an alternative.

What is the design of ClearCorrect cutouts?

We have two different designs for our cutouts; double vertical slits and half-moon button cutouts.


Can I request a single slit instead of a double slit?

No, ClearCorrect's double vertical slit design has been determined to work the best for our high and flat trimline.

Can I request a different angulation of the slits?

No, ClearCorrect's double vertical slit design has been determined to work the best for our high and flat trimline.

Is the single slit (or hook) design better than the double vertical slit design?

When simulating the wearing of elastics by a patient (opening and closing the mouth), for a 30-day treatment with daily exchange of elastic, showed that the double slit design is equivalent to the single slit design.

Will the double slit design increase the chances for the aligner to be less retentive (or pop out)?

When simulating the wear of elastics by a patient (opening and closing the mouth), for a 30-day treatment with daily exchange of elastic, showed that the double slit design is as retentive as the single slit design.

Will the double slit design deform or flare more easily than the single slit design?

When simulating the wear of elastics by a patient (opening and closing the mouth), for a 30-day treatment with daily exchange of elastic, demonstrated no statistical difference between the double slit and single slit design when it comes to deformation and flaring of the aligner.

Do I need to request more engagers with the double slit design to improve retentiveness vs. the single slit design?

When simulating the wear of elastics by a patient (opening and closing the mouth), for a 30-day treatment with daily exchange of elastic, demonstrated the double slit design is as retentive without engagers as the single slit design with engagers.

Can I customize the position of the button cutouts within the tooth surface?

You can request the technician to place the button in the center of the tooth or in a more mesial/distal position.

Can I place cutouts lingually and buccally?

By default, we will plan cutouts to be placed on the buccal side. However, we can plan and produce cutouts placed lingually. You can request this in the “Additional information” section of the prescription form in the Doctor Portal, or in the Comments Panel in ClearPilot™.  Please note that we won’t be able to place button cutouts on the lingual side on the anteriors (from canine to canine). We suggest that you use slit cutouts on the lingual side if you want to place them on the anteriors (from canine to canine).

Are there any limitations on the number of cutouts in a single aligner?

We don’t have any limitations on the number of cutouts that can be requested in a single aligner. 

Will cutouts be available to Orthodontists only?

No, any doctor, GP or Orthodontist will be able to request cutouts in their aligners.  

Will ClearCorrect provide the elastics, buttons, and related materials?

No, ClearCorrect will provide only the aligners with the pre-made cutouts.

I have a patient who has already started treatment, can I still request cutouts?

Yes, you can submit a revision and request cutouts in the aligners. Additional charges may apply based on the treatment option selected for that case and/or whether or not you have exhausted your available revisions.

Is there an additional cost for cutouts? 

No, cutouts are included within the cost of the treatment option of your preference.

Can I request to have the cutouts start at any step of the treatment?

By default, we plan cutouts to start on Step 5 through the end of treatment. You can request to change the start step to any other odd numbered step in the treatment. You can also modify the number of steps you want them to last. You can make these requests in the “Additional instructions” section of the prescription form in the Doctor Portal or through the Comments panel in ClearPilot™.

What should I do if I approved a treatment setup in ClearPilot that included a cutout(s), but the aligner(s) arrived without cutout(s) made?
